Send us a recent photo (no costumes please) and a letter telling us about your relationship, how long you've been together and why you need the makeover.
Be sure to include your name, address, day, evening and cell phone numbers.

Remember, participants chosen for a makeover must be available to appear on the show between July 9th and July 22nd, 2012.
