HOW TO PLAY:
Send in your "Summer" themed photo, Entry Information and
On Air Release for a chance to be selected to play the Trivia Game.
Each day, one contestant will be randomly selected for a chance to play "LIVE's Surf's Up Summer Trivia" on the show. If provided, the contestant's photo
may be featured on LIVE! when they receive a phone call from the hosts or they may have the option to live video chat with the hosts using Skype.
The hosts will spin a prize wheel to give the contestant the opportunity to either answer that day's trivia question about a previous show's broadcast or a
general question about the show.

After submitting your Online Entry, download and print this On Air Release; then complete, sign and scan the On Air Release and email it to Live@enteractivesolutions.com .
MAIL PHOTO ENTRY:
Legibly handwrite on a postcard (no larger than 5 x 7) your first and last name, date of birth, complete home address, daytime telephone number and your
Skype video chat username (optional) where you can be reached between the hours of 8:00 a.m. and 11:00 a.m. ET for regularly scheduled Show tapings. Then,
complete and sign an On Air Release; select your "Summer " photo (not to exceed 5" x 7"); and mail together in an envelope (no larger than 6" x 8") to:

Mail Photo Entries must be received by 12:00 noon PT on July 20, 2012.
NO PURCHASE NECESSARY. Void in Quebec and where prohibited. Open to permanent legal residents of the fifty states of the U.S., D.C. and Canada (excluding Quebec), who are 18 years of age or older as of the date of entry. Entries start on 6/15/2012. Internet Entries and Mail Photo Entries must be received by 7/20/2012 and Alternate Mail Entries must be received by 7/25/2012. On Air Releases are required from every entrant in order to be selected as a potential Show Participant for the Trivia Game.
